Iron deficiency anemia is commonly seen in patients with inflammatory bowel disease. However, endoscopy is time consuming and can be potentially harmful due to exposure to the duodenal contents. We sought to evaluate the efficacy and safety of the new Duet micro-endoscope for endoscopic evaluation of patients with iron deficiency anemia. We performed a prospective, single-center trial in patients with iron deficiency anemia who underwent a video capsule endoscopy with the Duet micro-endoscope. We analyzed the study time, the number of images, the time to reach the cecum, the total time of the procedure, the number of patients with anemia of unknown etiology (AUE), and adverse events. One hundred four patients (mean age, 48.8 years; 59% women) were enrolled. The mean study time was 9.9 minutes (range, 5-25 minutes) and the mean number of images was 61 (range, 10-180). All of the procedures were completed successfully, without complications. The mean time to reach the cecum was 7.6 minutes (range, 2-25 minutes), and was the same for all of the patients. The mean time to reach the terminal ileum was 17.8 minutes (range, 10-50 minutes) for patients with IBD and 8.6 minutes (range, 3-15 minutes) for those without IBD. The overall AUE rate was 12% (n = 13). In the first month, the AUE rate was higher in patients with previous abdominal surgery compared to patients without it (23.1% vs 7.1%, respectively; P =.047), and the AUE rate in patients with AUE was higher compared to patients without it (14.3% vs 2.8%, respectively; P =.005). The new Duet endoscope system was found to be a safe, feasible, and acceptable alternative to other available endoscopic platforms. It had a short examination time, was safe, and allowed for completion of the entire examination in 100% of patients. Our results suggest that Duet endoscopy is useful for diagnostic purposes in patients with iron deficiency anemia, particularly in those with previous abdominal surgery.Q:
